- Create new account in account history for FVG / $50 gap / Private fee
Steps to do DASS21
- Go to customercare@ gmail when patients finish
- Copy patient appointment in Dr. column to Reception column
- Change appointment type : DASS21 and place in WAITING
- Copy all the content in DASS21
- Go to View Record in appointment in Reception column
- Paste in Today’s notes
- Reason to Visit: DASS21
- Copy the appointment in the Reception column to the Dr. column, placing above the
current patient appointment with Dr. - Change appointment type to DASS21 DONE and placing in waiting
- Wait for doctor to call
Several times Visit (Existed Patient)
- Any changes in address or phone number?
- Still holding insurance?
- Check eligibility or COH if needed
- Check FVG
- Informed $50 gap fee if OVHC / CBHS / Private full fee
- Medicare holder: Medicare/Concession eligibility / FVG or informed full fee
Nurse appointment
- DO NOT HAVE FVG
- Always have Incentive number
- Vaccination will be separated with consultation fee
- Nurse always place patient under doctor name for consultation fee
- Practice – Practice for clinic for vaccination
Result appointment
- Can place under any doctor when booking
- Cannot answer the name of report
- Call patient for follow-up appointment for result discussion (Reception column)

- HICCAP – Transaction – Submit Quote – Swipe / Tap Insurance card – Provider – any
