New GP onboard checklist 

  1. Apply for Provider number 

Doctors can apply for a new provider number via their Proda account. If they cannot apply via Proda, they can fill out form HW019 

Information required from us are:

  • Clinic’s name: AIM Health Melbourne Student Medical Centre
  • Address: 253 Lonsdale St, Melbourne VIC 3000 (this is the address on the Medicare list, if they add Level 1 in front, it will show mismatch)
  • Organisation site ID: 2458678485 
  • If the new doctor needs to claim medicare, we also need to provide Minor ID/location ID
    • Location ID for AIM Student MD: HSS57292
    • Location ID for MCMC BP: BPS28499
  1. Create an account in MD or BP, set up a unique payment type
  1. Apply for HICAPS multi-merchant account

需要医生提供的资料有：

  • Name or Name of company
  • ABN
  • 地址
  • 电子邮箱
  • Mobile number
  • Bank details (bank name, acc name, BSB, acc number)
  1. Apply for HICAPS portal account

  1. Register with Medibank claiming portal
  1. Register with BUPA, fill out form add new provider 
  1. Add doctors to pathology/radiology direct downloading 
  1. Update reception tables/list
    1. Provider number list
    2. Payment type list
    3. Procedure booking list
  1. Set up appointment book
    1. Add practitioner in staff’s appointment book list
  1. Set up Health Engine booking
  1. Update Health Direct
  1. Update website information
  1. Order stamp and name card
  1. Set up surcharges in Hicaps (need at least completing 3 days of transition)
